• This page is for all JDW high school girls and guys who would like to join us for a day trip to Cascade Falls in Patapsco Valley State Park in Elkridge, MD on Saturday, September 10th, 2022.
  
  All teens MUST register here AND ensure that the teen ministry office has a Field Trip liability form on file at JDW before they can attend.
  
  The cost for this trip is $5/per teen.  You will be asked to bring a packed lunch with you and to wear the appropriate clothing for both hiking and swimming.  Once registered, the parents/teen will receive a detailed email with all information needed for the trip.

  • Everyone who attends this trip MUST have a permission form on file with the leaders or they cannot attend.  This form must be filled out in it's entirety by the parents and signed.  In case of any emergency, it is imperative that we have all information needed to give to any emergency personnel.  This is for the safety of the teen and the entire group.

  • For Payment Below:  You are able to pay by either credit card or check below.  If you cannot pay either way, you must bring in payment of $5/per teen as soon as possible to the teen ministry office (not parish office please).  Your teen/s will not be considered fully registered until we receive both payment and field trip liability form.
